Foreign Agents must submit an outline of ALL fees charged or potentially charged to participants. This must outline program fees that may cover other program costs as well (i.e. health insurance, reprinting of forms, etc.). If you do not charge participants for a service that is listed, please enter $0. If you only have one overall program/administrative fee, please include this in “Overall Program Fees.” If you have additional fees or discounts, not listed such as “early bird specials”, etc. refer to the "Additional Fees & Discounts" portion of the document and include description details.
